Suvera

Suvera

Suvera delivers a proactive Virtual Clinic that combines technology with compassionate, community-based care.

  • More information

How does this innovation meet neighbourhood health priorities?

Suvera’s mission aligns closely with the Neighbourhood Health vision of bringing care closer to communities and empowering people to live healthier lives. Working with PCNs and GP practices across England, we deliver a proactive Virtual Clinic that combines technology with compassionate, community-based care. With every partner we work directly at practice level: getting to know their workflows, operational processes, and the local organisations or support groups they want us to connect patients with. For example, in partnership with one ICB, we’ve co-designed a programme that links patients to community charities and health coaching services, helping them manage long-term conditions more holistically.

Our population health and recall tool, Planner, frees up capacity by automating workflows and identifying patients most in need, ensuring resources are directed where they have the greatest impact. This approach prevents adverse events, keeps people out of hospital, and improves patient access – all while saving appointment time and admin hours. In line with Neighbourhood Health, Suvera is showing how digital innovation and community collaboration can transform primary care and create healthier neighbourhoods.

    User quote

"I have been using Suvera's services for a few years now and have found their innovative methods of helping patients fantastic… a mature understanding of practice QoF work as well as the PCN Network DES and used it to roll out online services to help practices and PCNs achieve higher prevalence and better monitoring for long term conditions.  I look forward to more innovative ways in which Suvera will help general practice in the future"

Heidi

Heidi

Clinicians lose valuable time to documentation - Heidi gives it back with automated clinical notes.

  • More information

How does this innovation meet neighbourhood health priorities?

The neighbourhood model succeeds when teams share consistent, contemporaneous clinical records that make cross-service working effortless. Heidi Health is built for that reality. Our platform ensures high-quality, contemporaneous consultation documentation and structured records that can be configured and customised to the needs of GPs, multidisciplinary teams, allied health professionals, mental-health services and community partners.

That balance of consistency and configurability lets neighbourhood teams capture the right detail for each discipline while preserving shared data for continuity, handovers and coordinated care. By making documentation usable, timely and adaptable to local models, Heidi helps neighbourhoods operate as integrated systems and deliver community-centred outcomes.

    User quote

"Heidi is genuinely incredible. This is going to be the future. Not only does it save time, but all aspects of the consultation are recorded allowing for more accurate notes. It also allows you to be fully engaged with patients, rather than intermittently having to tap away at notes. Genuinely shocked at how good it is."

How does this innovation meet neighbourhood health priorities?

“The Hope Programme strengthens neighbourhood with practical, holistic, evidence-based self-management support close to home. It is co-designed and delivered by people with lived experience of long-term conditions, making it grounded in what matters to people. Hope Programme’s flexible digital and in-person options mean everyone can take part. User quote

“HOPE is a very well presented and thought-out course. I love that it's in small sections so that on a bad fatigue day I could still achieve a little something by completing a session to chip away at the overall load. This has given me the confidence to try another course.”

Asclepius Surgfit

Asclepius Surgfit

Surgfit™'s wearable biosensor and patient App uniquely captures physiological and functional capacity data to determine patients’ fitness for surgery.

  • More information

How does this innovation meet neighbourhood health priorities?

Asclepius MedTech has reimagined how hospitals deliver surgical preoperative assessment services with its "groundbreaking" technology Surgfit™.

Clinically trialled its wearable biosensor + patient App uniquely captures physiological and functional capacity data to determine patients’ fitness for surgery

Surgery is often seen as safe with full recovery expected, but it carries high risks. Despite advances in surgical and perioperative care, complication and readmission rates remain unchanged.

    1. 30% of patients experience complications that may cause lasting problems.
    2. 1 in 10 surgical patients are readmitted costing the NHS £13.7billion annually.
    3. 30% of patients miss their pre-op due to health inequalities.
    4. Waits lists and cancellation rates remain challenging high.

Clinical consensus is that current assessment technologies to assess patient fitness for surgery are subjective, unreliable, costly and inconvenient and hospital based.

New tools and models of care are needed to improve surgical care, address productivity in the NHS 10-Year Plan and enable virtual surgical pre-op assessment in patients own homes.

Asclepius welcomes the Grow Neighbourhood Health Innovation Fund. Working with Integrated Care Systems (ICS) to explore initiation of Surgfit™ in GP Surgeries, Pharmacy or Care Homes and deliver pre-op assessment closer to where people live and improve surgical outcomes.

    User quote

"As former Deputy CEO of NHS England I am only too aware of the pressures on the NHS. Surgfit offers a significant opportunity to bring preoperative assessment into the modern world, using its digital capability and remote working to increase efficiency, whilst tackling the growing numbers of patients on elective surgical waiting lists"

AirEmail

AirEmail

Protecting NHS patients and staff from email overload, giving staff time back to care and share, making email manageable and safe using intelligent automation (IA).

  • More information

How does this innovation meet neighbourhood health priorities?

Neighbourhood Health depends on coordination across multidisciplinary teams and care pathways spanning general practice, community, and secondary care. Healthcare email provides unified communication accessible to all staff regardless of EPR system. Yet NHS staff are overwhelmed - 1 in 3 find email unmanageable, 1 in 2 clinicians report missing clinical emails (Bakhai, 2022), with an estimated 19 million clinical emails unread annually (NHS DigiWell study, 2024).

AirEmail supports Neighbourhood Health implementation by improving communication reliability across Integrated Neighbourhood Teams, between primary and community services, and during transitions of care. Our healthcare email management tool, validated in an NHS clinical trial, helps clinicians stay on top of clinical communications - particularly vital for 'Home First' approaches where patients move through complex pathways across different care settings, and for urgent neighbourhood services where timely communication can prevent escalation.

For patients navigating complex care pathways across multiple neighbourhood providers, AirEmail reduces the risk of emails being missed. This is particularly important for vulnerable patients, who are less likely to chase their care, and therefore more at risk of falling through gaps between services.

    User quote

"Prioritising clinically important emails amongst the volume is essential. AirEmail has made me a lot safer in terms of missing patient emails. I love the highlighter function – it enables you read emails quickly."

ApptHealth

ApptHealth

A digital health platform that automates care coordination for people living with multiple long term conditions.

  • More information

How does this innovation meet neighbourhood health priorities?

Appt’s innovation aligns directly with the goals of Neighbourhood Health and Modern General Practice by championing patient choice, streamlining the end-to-end journey and aligning capacity with need. With Appt, proactive planned care becomes scalable and reliable, ensuring no one is left behind.

Appt’s digital platform acts as a virtual care coordinator, safely matching each patient’s care needs to available appointments and appropriately skilled staff across local practices. People living with multiple conditions often spend hours coordinating their own care, which can lead to confusion, delays and overwhelm. With Appt, patients are able to choose how they prefer to engage, through the NHS app, text message, emails, letters or automated phone calls, ensuring that access is equitable and flexible.

The innovation reduces the time patients, carers and staff spend on care coordination, and increases practice capacity by consolidating multiple health screening appointments into a single, well-planned visit. This reduces the burden on patients to self-manage complex journeys, whilst practice teams are more productive and spend less time identifying who needs care, locating suitable appointments and contacting individuals.

    User quotes

"The implementation has been one of the most successful I’ve experienced in the NHS. Appt has had a hugely positive impact on our programme."

    - Stephen Lees, Program manager, Lung cancer screening West Yorkshire.

"Appt's determination to disrupt preventative healthcare and consistently deliver with excellent acknowledgement of feedback is impressive."

    - Osman Bhatti, Clinical Information Officer, North East London CCG
