The EMAHSN stroke rehabilitation programme was commissioned from 2014 - 2016. The resources developed as part of this programme are relevant to the ambitions set out in the NHS Long Term Plan.
The aim of the programme was to reduce inequality of care provision to ensure that stroke survivors and their families are supported in their recovery journey.
The main areas addressed were:
Evidence-based service specifications for stroke early supported discharge and community rehabilitation were created by using consensus documents developed by the team at the University of Nottingham and collaborative working with stakeholders.

Guidance was also created relevant to the 2019-20 CQUIN: six month review for stroke survivors.

A comprehensive report about stroke service provision across the East Midlands was also disseminated to all East Midlands Clinical Commissioning Groups. View the Provision of Care Pathways for stroke survivors across the East Midlands' report.
The team led region-wide workshops to improve consistency and alignment of data collection by Early supported discharge (ESD) and community stroke services to promote performance monitoring. Helpnotes were created in collaboration with the Sentinel Stroke National Audit Programme.
A bespoke programme of workshops across the East Midlands was delivered to promote evidence-based practice.
