Supporting our systems to improve patients care and outcomes by effectively identifying and treating patients with hypertension (high blood pressure).

HIEM will work with ICB’s, NHS Trusts and Primary Care Services across the East Midlands. Specifically, it will aim to accelerate the identification and optimised management of those with hypertension. This includes using innovative approaches and novel medications to supplement current clinical assessment processes, underpinned by changes in pathways, and data collection.

Integral to success is the methodical application of the NICE Guidance and Proactive Care Frameworks for Hypertension.
