Supporting our systems to improve patients care and outcomes by effectively identifying and treating patients with secondary prevention hypercholesterolaemia (patients with high cholesterol after a CVD event such as a heart attack or stroke).
HIEM will work with ICB’s, NHS Trusts and Primary Care Services across the East Midlands. Specifically, it will aim to accelerate the identification and optimised management of those with hypercholesterolaemia post CVD event. This includes using innovative approaches and novel medications to supplement current clinical assessment processes, underpinned by changes in pathways, and data collection.
Integral to success is the methodical application of the NICE guidelines (NG238) as well as the national guidance for lipid management in relation to primary and secondary prevention of CVD.
