Delivery of a national programme designed to strengthen how learning from patient safety incidents is translated into meaningful improvement, is being led jointly by Health Innovation East Midlands and Health Innovation West Midlands through the Patient Safety Collaborative (PSC) network.

 

The organisations are coordinating the national PSIRF Insight to Improvement Programme, providing leadership, resources, methods and support to all 15 Health Innovation Networks (HINs) across England. The initiative supports NHS-funded organisations in developing stronger approaches to learning from patient safety events and using those insights to drive sustainable improvements in care.

The programme sits alongside the implementation of the Patient Safety Incident Response Framework (PSIRF) and aims to build a clearer understanding of how organisations move from identifying patient safety issues to delivering measurable improvements. It also seeks to identify the infrastructure, capabilities and conditions that enable effective learning and improvement across healthcare settings.

By bringing together intelligence from organisations nationwide, the programme will help create a national picture of emerging patient safety priorities, current improvement activity, and the factors that enable or constrain progress.

Structured support for NHS providers

Participating organisations are supported through a structured diagnostic approach designed to assess and strengthen how patient safety learning is translated into improvement. This includes:

  • Reviewing Patient Safety Incident Response Plans (PSIRPs) to identify and collate organisational patient safety and improvement priorities.
  • Mapping patient safety governance arrangements alongside the flow of insight, learning and improvement arising from patient safety events.
  • Facilitating provider self-assessment of the development and adoption of system features that support effective implementation of PSIRF.

Through this process, organisations can identify strengths, gaps, barriers and opportunities for future development. The programme also enables Health Innovation Networks and national workstream leads to identify common themes, share effective practice and strengthen the evidence base for patient safety improvement across England.

Health and care organisations invited to take part

NHS providers are being encouraged to participate in the programme and access tailored support from their Health Innovation Network team.

By aligning local improvement efforts with the wider national programme, organisations will also contribute to a growing evidence base that supports learning and improvement across the NHS.
