The work required is to deliver an evaluation of the use of a managing deterioration tool in a home care setting, enabled by digital technology, to support the safe identification and response to acute deterioration in care recipients by sharing structured warning sign observations with primary care.

Bidders should return the ITQ with their offer detailed in section 2, along with all appendices and any supporting documents.
