Join this webinar to learn about the 'LUCID - Connecting Kidney Care' programme and its role in supporting earlier identification and management of chronic kidney disease (CKD) through integrated primary and secondary care. The session will provide an overview of the scale and impact of CKD, the evidence base behind the LUCID approach, and how local delivery aligns with national priorities including the NHS 10 Year Health Plan and the recent Cardiovascular Disease Modern Service Framework. Attendees will also hear about the support available through the Health Innovation Networks (HINs), and have the opportunity to ask questions and discuss implementation considerations with the programme clinical leads: GP Dr Melanie Morris and Consultant Nephrologist Dr Rupert Major.
We will be holding two identical sessions: the first will be on Thursday 29th October 1pm-2pm, and the second will be on Tuesday 3rd November 1pm-2pm.
This session is aimed at primary care clinical and operational teams, secondary care nephrology teams, and ICB CVD/CVKM teams.
