East Midlands Patient Safety Collaborative (EMPSC)

Patient Safety Collaboratives (PSCs) are hosted by AHSNs with funding from NHS England and NHS Improvement. They deliver a series of National Patient Safety Improvement Programmes which are key elements of the NHS Patient Safety Strategy. Our work helps reduce error, harm and death which can result from failures in the system and enhances safe, quality care – the national ambition is for the NHS to become comparable with the safest health care services in the world by 2025.

Our East Midlands Patient Safety Collaborative (EMPSC) plays an essential role in identifying and spreading safer care initiatives, working alongside Integrated Care Systems (ICSs), provider collaboratives and wider health and care partners across the region.

During 2021-22 our work focussed on the following areas:

  • Reducing the rate of stillbirths, neonatal death and brain injuries occurring during or soon after birth.
  • Reducing harm associated with physical deterioration in adults and children.
  • Reducing avoidable severe medication- related harm.
  • Improving the safety and outcomes of inpatient mental healthcare.
  • Supporting the spread and adoption of targeted discharge care bundles.

During the peak of the COVID-19 pandemic, our team played a key role in the response by supporting system partners – for example supporting care homes and the rollout of Covid Virtual Wards. The team continues to be integral to the recovery of services across safety priorities.

Impacts

Connecting Together care home webinars: designed to help care home staff and managers stay connected across the region. Received interest from 330 care home staff – who can access resources on good practice in improving quality for residents.

We worked with five mental health trusts and two private providers to reduce incidences of restrictive practice in inpatient wards. This includes 24 wards spanning 11 different specialities.

Working alongside the West Midlands PSC and East and West Midlands Association of Directors of Adult Social Services (ADASS), we ran a pilot engaging four local authorities and Clinical Commissioning Groups to support 13 learning disability supported living providers across the Midlands. This trained over 100 carers to use deterioration identification tools and escalation tools, to improve health outcomes of people with learning disabilities living in supported living settings.

Delivered quarterly safety network events with shared learning of improvement work with engagement from five Local Maternity Neonatal Systems. Alongside the West Midlands PSC we facilitated webinars to support the delivery of a whole pathway focusing on the optimisation and stabilisation of the preterm infant, along with an appreciative inquiry workshop to support an improvement culture.

Next Steps

For 2022-23 the Patient Safety Collaborative will be working closely with each ICS to develop local patient safety networks to address ICS safety priorities alongside the delivery of the national safety improvement programmes.
